SAFETY FIRST - Make sure to review the safety guidelines each time you use a station.
ASK FOR HELP - If you have any questions or concerns, ask an advisor for help!
BE A GOOD NEIGHBOR - Creation and collaboration can get LOUD. Be mindful of the people around you.
CHECK YOUR CERTIFICATIONS - Make sure you have completed training before operating machinery.
SHARE SPACE RESPECTFULLY - Treat others as you want to be treated and respect equipment as if it were your birthday gift, so we can all enjoy it!
CLEAN SPACES ARE HAPPY PLACES - Leave an area as you found it (or better). Clean-up after yourself, and be aware of any materials or tools that you could help quickly put away/throw away/wipe down.
COMMUNICATE & WORK TOGETHER - Share your ideas, communicate your needs and boundaries, and collaborate with your creative companions!
CREATING IS A TEAM EFFORT - Make sure your project plans from start to finish aren’t going to negatively affect anyone else.
BREAK OUT OF YOUR COMFORT ZONE - Investigate what the space has to offer, and what other Questers are interested in exploring with you!
HAVE FUN! - We learn best when we are having fun. Find what you enjoy and innovate, create, and design something spectacular!

We will not allow any personal technology devices, as they can not be restricted/controlled in the environment. If a Quester shows up with a device, it will be left in their locker for the duration of the day.
Questers will have access to laptops for use during their research, using machine applications, and utilizing how-to videos or tutorials. Questers are not allowed to idly watch or listen to anything online, unless it directly relates to what they are creating.
Before morning meeting, and after clean-up (particularly during after-care), Questers are allowed to play some games on the laptops, but are strongly encouraged to play socially. We also have a Nintendo Switch available for social and active gaming. This will be utilized more in After-Care, but also may provide some breaks (in moderation). All gaming will be done in a supportive environment, and the kids will be encouraged to create gaming tournaments to include everyone who is interested in playing, no matter what level they may be!

The nature of our makerspace and child-led environment requires participants to be responsible for their own actions. Rules will be discussed each day a new Quester joins the group, and as needed in our afternoon check-ins.
Inside Side Quest Social Club I will follow the rules to keep the space safe, supportive, and fun. These rules will include safety guidelines for equipment, working together, cleaning the space, technology/application usage (but no personal devices), using words to encourage and support each other, etc.
Outside Side Quest Social Club, I will walk calmly near streets, listen to staff instructions, and pay attention to my surroundings to keep myself and others safe.
I will work with others cooperatively to create a mutually respectful environment.
I will not disturb or hurt others, verbally or physically.
I will not place myself or others in a dangerous situation.
I will not damage equipment or property.
If challenges arise where the participant is not meeting expectations for a safe and mutually respectful environment, one or all of the the following may occur:
Meeting directly with staff to discuss expectation challenges, using a problem-solving protocol.
Missing out on activities, projects, or field trips.
A call home or meeting with the parent/guardian to discuss behavior.
Dismissal from the Summer Club program.
- Before this decision is made, parents will be engaged in problem-solving discussions. After all other possibilities are exhausted, a participant may be asked to leave our Social Club. No refunds will be granted.

If your child(ren) have any symptoms (runny nose, coughing, lethargy, stomach upset, headache, etc) while at Side Quest, we will take their temperature with a forehead thermometer. We will contact an adult for pick-up ONLY if temperature is above 100°F.
For COVID-19, if your child tests positive, please keep them home until they are fever free for 24 hours. If they still have some symptoms when returning, we will ask them to wear a mask.
We will notify adults of any communicable illness, so that they can make the best choice given their families needs.
General Guidelines:
- A child should stay home if they have a fever of 100°F or higher, and can return after 24 hours without a fever.
- A child should stay home if they have vomited or have diarrhea more than twice in 24 hours.
- A child should stay home if they have a contagious disease, such as chickenpox, strep, flu, COVID-19.
- A child should stay home if they have trouble breathing, signs of weakness or tiredness, or a new rash. -
This health history is correct and accurately reflects the health status of the attendee to whom it pertains.
The person described has permission to participate in all activities except as noted by me on this form, or in writing to hello@sidequestsocial.club after the form is submitted.
If I cannot be reached in an emergency, I give my permission to the physician to hospitalize, secure proper treatment for, and order injection, anesthesia, or surgery for this child.
I understand the information on this form will be shared on a "need to know" basis with camp staff. I give permission to photocopy this form.
In addition, Side Quest Social Club has permission to obtain a copy of my child's health record from providers who treat my child and these providers may talk with the program's staff about my child's health status.

As a parent or guardian, I understand that my child will not be released to anyone that is not listed on this form as a Parent/Guardian or Emergency Contact.
I understand that Side Quest Social Club Staff may ask to see a form of identification before releasing my child to verify the names on file.

I agree to follow all rules and regulations of Side Quest Social Club LLC while in, upon or about the premises or while using or observing the premises or any facilities or equipment, or participating in any program affiliated with Side Quest Social Club LLC without respect as to location, and understand and agree that I may be expelled at any time, with no refund of any monies paid, for failure to abide by such rules and regulations.
IN CONSIDERATION OF BEING PERMITTED TO UTILIZE THE FACILITIES, SERVICES AND PROGRAMS OF SIDE QUEST SOCIAL CLUB LLC FOR ANY PURPOSE, INCLUDING BUT NOT LIMITED TO OBSERVATION OR USE OF FACILITIES OR EQUIPMENT OR PARTICIPATION IN ANY PROGRAM AFFILIATED WITH SIDE QUEST SOCIAL CLUB LLC WITHOUT RESPECT AS TO LOCATION, I HEREBY AGREE TO THE FOLLOWING:
1. I UNDERSTAND THAT ACTIVITIES AT THE FACILITY OR ELSEWHERE, INCLUDING USE OF EQUIPMENT AND PARTICIPATION IN PROGRAMS, CAN INVOLVE MOVEMENT, STRAIN AND OTHER ELEMENTS THAT CREATE RISK OF SERIOUS INJURY OR DEATH. I ALSO UNDERSTAND THAT PROGRAM ACTIVITIES INCLUDE FIELD TRIPS TO LOCATIONS OUTSIDE THE SIDE QUEST SOCIAL CLUB LLC PREMISES, AND THAT PUBLIC OR PRIVATE TRANSPORTATION MAY BE UTILIZED TO TRANSPORT PARTICIPANTS TO AND FROM THESE FIELD TRIP LOCATIONS. I HEREBY ASSUME FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE OR LOSS, regardless of severity, that I or my minor child/ward may sustain from my or my minor child/ward’s presence in, upon or about the premises or while using or observing the premises or any Facilities or equipment, or participating in any program affiliated with Side Quest Social Club LLC without respect as to location, or while being transported to and from field trip locations outside the Side Quest Social Club LLC premises, except for any injury damage or loss that is caused solely by Side Quest Social Club LLC’s gross negligence.
2. I, FOR MYSELF, ANY PERSONAL REPRESENTATIVES, ASSIGNS, HEIRS AND NEXT OF KIN, HEREBY FULLY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE SIDE QUEST SOCIAL CLUB LLC, its operating centers, their respective officers, directors, Board of Managers, Trustees, members, volunteers, employees or agents (the “Releasees”) and each of them from any and all claims for injuries, damage or loss that I or my minor child/ward may incur whether in, upon or about the premises or while using or observing the premises or any facilities or equipment, or participating in any program affiliated with the Side Quest Social Club LLC premises, except for any injury damage or loss that is caused solely by Side Quest Social Club LLC’s gross negligence.
3. I HEREBY AGREE TO INDEMNIFY AND HOLD HARMLESS the Releasees and each of them from any loss, liability, damage or cost they may incur from my or my minor child/ward’s presence in, upon or about the premises or while using or observing the premises or any facilities or equipment, or participating in any program affiliated with Side Quest Social Club LLC without respect as to location, or while being transported to and from field trip locations outside the Side Quest Social Club LLC premises, except for any loss, liability, damage or cost that is caused by Side Quest Social Club LLC’s gross negligence.
I further expressly agree that this Agreement is intended to be as broad and inclusive as is permitted by the law of the State of Illinois and if any portion thereof is held invalid, it is agreed that the remaining Agreement shall, notwithstanding, continue in full legal force and effect.
THIS AGREEMENT APPLIES TO ALL PAST, PRESENT AND FUTURE VISITS AND USES BY ME TO ANY SIDE QUEST SOCIAL CLUB FACILITY OR PROPERTY.

I hereby grant Side Quest Social Club LLC the right to create via photography or other means, and the right to reproduce, display, and disseminate worldwide and in perpetuity, in any traditional or electronic media format, such photographs or other image of my or my minor child/wards likeness.
I understand that these images will be used in an appropriate and respectful manner. I confirm that these images were taken with my knowledge and consent.
I understand that my minor child/ward's face(s) will NEVER be shown without additional written consent of a parent/guardian AND the child regarding the specific instance.

If you need to cancel for any reason, please let me know. I will try my best to get your kiddo(s) into another week if something comes up! Our club is intentionally small to provide the best experience, but that also means, we have a little wiggle-room when it comes to shifting schedules around to accommodate last-minute life-happenings.
If you must cancel, our cancellation policy is as follows:
- More than four weeks: 10% administrative fee
- Between four and two weeks: 50%
- Less than two weeks: 100%
